When you exercise, your blood pressure climbs. That is normal. How much it climbs turns out to carry information. This review gathers evidence that an outsized rise during exercise, or a blunted one, predicts higher cardiovascular risk, sometimes before a resting reading looks abnormal. The most telling signal appears during submaximal effort, not all-out exertion, and it means more when judged against how hard the body is actually working.
The encouraging part: this marker can move. People with higher cardiorespiratory fitness, the capacity to sustain hard effort, tend to show a healthier exercise blood pressure, and both aerobic and resistance training can nudge it down. That reframes exercise blood pressure not as a fixed trait but as something you can improve. We argue it belongs in routine exercise testing.
